I threw together a list of puzzlehunt team names, gathering names from a bunch of old game signup sheets and results pages. I didn't try to "combine" clusters like { Burninators, The Burninators, The burninators } into one team (though I did kinda try to sort them close to each other).
(I'm trying to figure out: Suppose that the 2-Tone Game, instead of using OpenID, encouraged you to create a team account. What kinds of names would people choose? If the game didn't let you use a % symbol in your team name, would that limit teams' expression? (Not many.) How about forbidding the & symbol? (Now you got troubles.)) (This list doesn't help me figure out: What happens when the NYC team Meat Machine finds out that the Hot Springs team Meat Machine already grabbed the name "Meat Machine"?) (Anyhow.)
